The role of the ID Support/Outreach & Testing Staff is to provide administrative/clerical support to the Infectious Disease team, especially case management. This person will need to organize and maintain all ID case management records, in both electronic and paper form; assist in coordination of patients’ transportation and appointments; provide follow up when patients miss appointments; and, assist case managers and provider, as needed. In addition, this individual will be responsible for coordinating the Emergency Room testing with HRMC; planning and organizing other outreach, testing and educational activities; and, providing information and guidance for individuals requesting PrEP.

Duties:
- Organizing and maintaining all ID case management records (Intakes, Assessments, ADAP applications, etc.).
- Assisting with gathering information and completing necessary forms, as required by RW programs.
- Assist case managers in ensuring that all ADAP client applications are completed and renewed according to the program guidelines.
- Provide a system for organizing all patients’ records appropriately. Appropriately maintaining all records in eCW and on paper.
- Communicating effectively and professionally with patients and internal/external resources by phone and in writing.
- Assisting case managers in ensuring that all patients have adequate access to primary HIV care.
- Coordinating patient care appropriately by scheduling/rescheduling appointments and receiving/scheduling referrals.
- Work with RHG’s case management team to maintain a network of support for the target population
- Responding to inquiries about PrEP properly and providing the necessary guidance and assistance for appropriate candidates.
- Collaborate with Halifax Regional Medical Center on Emergency Room Testing.
- Adequately complete process for gathering blood samples for testing.
- Perform any program specific data entry and complete grant required reports as directed by the program coordinator.
- Communicating with hospital (HRMC) and State/Network partners to ensure that program is effective. Establishing and maintaining a good rapport and open communication with partners.
- Coordinate and participate in Outreach activities including working with media outlets to increase community awareness. Setup and schedule outreach activities.
- Assist in providing education and raising awareness in the community about HIV, PrEP, ED Testing and associated topics.

- • Bachelor's level degree strongly preferred (preferably BSW, BSN, or other health or human services related degree)
