The Infectious Disease (ID) Program Coordinator position will oversee a team of staff that provides both medical case management and outreach services for individuals who are HIV and Hepatitis C positive. The ID Program Coordinator collaborates with the patient, the health care provider and other health/social service agencies to enhance patient-centered care that maximizes outcomes across the patient care continuum. Case management services may include patient advocacy; monitoring patient care; addressing patient and family needs; resolving obstacles to effective care; coordinating services; maintaining case management records (electronic and paper form), assisting with treatment adherence; and, providing services to the patient, both in-house and in the community. In addition, as the ID Program Coordinator, this individual will be responsible for administrative oversight and ensuring that the program maintains compliance with Ryan White guidelines, as well as the requirements of regulatory agencies, funders and other community partners.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
- Ensures that all HIV clients (PLWHA) have access to primary health care. Coordinate case management process from patient’s entry into care to follow up care.
- Coordinates care and resources with physicians/other medical providers, social workers, other case managers, community resources and other team members to achieve optimal patient outcomes.
- Assess needs of patient/family. Develop care plan with patient’s input. Use care plan to implement appropriate patient care & case management goals.
- Appropriately identifies patients who need case management, based on the complexity of their needs.
- Recruit at-risk population in need of knowledge and skills to prevent and/or reduce chronic disease complications.
- Monitor patient progress, compliance and access to care. Provide follow up to those lost to care.
- Prepare progress reports and action plans. Assure that HAB measures for each client are met
- Supervise RHG’s case management team to create a network of support for the target population.
- Collaborate and communicate appropriately with the care team to ensure patient is receiving adequate, quality care.
- Assist in providing information and coordination of entry into care to patients requesting PrEP (Pre-Exposure Prophylaxis).
- Document ALL case management services provided for each HIV and Hep C client in eCW (RHG’s Electronic Medical Record). Also, document HIV services in State’s CAREWare system.
- Ensure program compliance with Ryan White and all grant and regulatory agency requirements.
- Participate in Regional and State HIV/AIDS Program meetings.
- Manage program specific data entry and complete grant required reports.
- Other duties as assigned.

REQUIREMENTS
- Associates or Bachelor’s degree in related field with appropriate related experience.
- Experience working in the healthcare field and working with grant coordination. Demonstrated leadership skills.
- Must have experience with outreach, low-income families, diverse communities, community/migrant health.
- Must be able to protect sensitive confidential patient information.
- Must have strong organizational skills and the ability to work independently.
- Possess and demonstrate critical thinking skills and problem solving techniques.
- Communicates effectively with all levels of personnel and both internal and external customers, deals with difficult situations diplomatically, develops and maintains collaborative relationships.
- Possess knowledge of modern office equipment, systems and procedures. Knowledge and experience working with Microsoft Office Professional. Experience working with large databases preferred.
- Must have exemplary oral and written communication skills. Must possess appropriate interviewing and negotiating skills.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
- Ability to write routine reports and correspondence. Ability to speak effectively before groups of patients, RHG employees or the public.

- High school diploma or GED required.
