The Population Health Specialist is dedicated to improving health outcomes for specific populations through data analysis and preventive care strategies. This role involves assessing health trends and disparities, collaborating with various stakeholders to address the social determinants of health that impact overall well-being. The ideal candidate will possess strong analytical skills, the ability to clearly communicate data and trends to promote health outcomes, and a passion for promoting health equity, particularly among underserved or high-risk groups.
Position may require travel within the community for meetings and collaboration efforts.

RESPONSIBILITIES

  • Data Analysis:
    • Analyze population health data to identify trends, disparities, and specific health needs within targeted communities.
    • Prepare reports and presentations summarizing findings and recommendations for program development.
  • Collaboration with Quality Improvement Specialists:
    • Work closely with Quality Improvement Specialists to create and implement plans aimed at improving workflow performance and patient outcomes.
    • Participate in the development of quality improvement initiatives to enhance service delivery and ensure compliance with healthcare regulations.
  • Patient Engagement:
    • Utilize reports to outreach to patients and schedule appointments, ensuring that individuals are connected to the services they need.
    • Understand scheduling requirements and maintain appropriate phone etiquette when engaging with patients to promote health literacy, awareness of available services, and access to care.
    • Conduct outreach efforts to educate individuals on preventive measures, chronic disease management, and health resources.
  • Outcome Monitoring:
    • Monitor health outcomes for defined populations, analyzing the effectiveness of implemented programs and making adjustments as necessary.
    • Utilize qualitative and quantitative data to measure program impact and inform future strategies.
  • Value-Based Care Initiatives:
    • Attend Value-Based Plan meetings to collaborate with stakeholders and align initiatives with organizational goals.
    • Review reports related to population health performance and quality metrics to ensure continuous improvement.
    • Address insurance patient attribution lists to ensure accurate tracking of patient populations and care coordination.
  • Reporting:
    • Provide regular updates and reports to the QI Director on population health initiatives, outcomes, and performance metrics.
    • Upload necessary documentation to payers' portals to close care gaps and ensure proper care coordination.
    • Assist in the preparation of reports for internal and external stakeholders regarding program effectiveness and health outcomes.

    • Foster a culture of continuous improvement in population health practices through evidence-based approaches.

REQUIREMENTS

  • Education:
    • Bachelor’s degree in Public Health, Nursing, Health Administration, or a related field.
  • Experience:
    • Minimum of 1-2 years of experience in population health, public health, or a related field.
    • Experience with data analysis and quality improvement initiatives is highly desirable.
  • Skills:
    • Strong analytical and problem-solving skills, with proficiency in Excel.
    • Excellent communication and interpersonal skills to effectively engage with diverse populations and stakeholders – internal clinical teams, practice managers, and operational teams.
    • Ability to clearly communicate data and trends to promote health outcomes.
    • Knowledge of healthcare systems (HER), social determinants of health, and health equity principles.
    • Ability to work collaboratively in a team environment and manage multiple projects simultaneously.
    • Understanding of scheduling requirements and appropriate phone etiquette for patient outreach.
    • Ability to apply common sense understanding when carrying out detailed written or oral instructions
    • Possess knowledge of modern office equipment and Microsoft Office software programs
    • Bilingual (English/Spanish) is a plus, but not required; applicants who may serve in a translating capacity will be required to take a language assessment
